A Level 3 Adult Foster Home (AFH) designed to accommodate up to five residents

Remodeled in 2025 to accommodate those with disabilities. This includes wheelchair ramp, fire sprinkler system, and ADA bathrooms with roll-in shower.

Each bathroom is stocked with essential hygiene supplies. Residents are welcome to use their own personal products if they prefer.

Each resident is provided with a fully furnished private room, including linens, internet/Wi-Fi, a call light, and a television. Residents are welcome to personalize their space with their own furniture, with staff approval.

We are a team of registered nurses who have come together to offer our community an alternative to traditional assisted living facilities. We welcome individuals who need assistance with activities of daily living (ADLs) due to disability and provide care in a warm, home-like environment. We strive to create a peaceful home where every resident feels safe, respected, and valued by both staff and fellow residents. We protect our residents’ rights, support their independence, and honor their decisions regarding health and care. In addition to collaborating with the resident’s healthcare team we also take time to learn the resident’s life histories, cultures, and preferences when planning their day-to-day care.
